Why ask about photographs?
Academies post performances and matches, and parents increasingly want a say. A yes-or-no on the form, signed by a parent, settles it before the first photo is taken.
Can it cover more than one discipline?
Yes. Rename the discipline field, or change it to “tick any” and list your disciplines as choices.
Where is my form saved?
Only in this browser, on this device, logo included. Nothing you type is sent to iVake. To keep a copy, print it and choose “Save as PDF”.
