Why ask who may collect the child?
A preschool hands a child only to an adult the parents have named. Writing two names and mobile numbers on the form, signed by a parent, gives the teacher at the gate something to check against.
Should we ask about immunisation?
Most preschools do, because young children share everything. The form asks only whether it is up to date and for the record as an attached document; change it if your policy differs.
Where is my form saved?
Only in this browser, on this device, logo included. Nothing you type is sent to iVake. To keep a copy, print it and choose “Save as PDF”.
