For most tutors, yes: who the student is, which subjects, which batch, and how to reach a parent. Add a field if you need more, such as fees or a start date.
Can a student take more than one subject?
Yes. Subjects is a “tick any” field, so a student can tick as many as they take.
Where is my form saved?
Only in this browser, on this device, logo included. Nothing you type is sent to iVake. To keep a copy, print it and choose “Save as PDF”.
